Von 1898 bis 1908 ist Landsteiner Assistent in der Pathologischen Anatomie, wo er 1900 die Entdeckung macht, dass das Blut zweier Menschen häufig verklumpt.
Im Laufe der Jahre entwickelt er daraus das AB0-System.
Nach seiner Zeit an der Universität wird er Vorstand des Wilhelminenspitals in Wien, was er bis 1919 bleibt.
